In-network Coverage

Flourishing Affinity Counseling is only in network with Blue Cross and Blue Shield PPO plans. Please note, FAC is not in network  HMO plans. As a courtesy, FAC will complete a benefit check to provide you with an insurance quote and an estimate of service fees. 
 
While FAC tries our best to obtain accurate information from your insurance prior to your first appointment, a quote of benefits is not a guarantee of coverage. Benefits are subject to all contract limits and the member’s status on the date of services. Accumulated amounts, such as deductibles, may change as additional claims are processed. Only after we submit the first claim and hear back from your insurance will we know for certain. You are always encouraged to contact your insurance by calling the number on the back of your card prior to your first appointment.

FAC also accepts all major insurance plans who have out-of-network coverage. FAC will help you navigate insurance reimbursement from your provider.

Question 4

Sliding Scale

FAC reserves limited spots for reduced fees to those who are eligible. Eligibility is determined by completing a reduced fee application and submitting the required supporting documents. Reduced fees range from $130-$185. We also offer a significantly reduced sliding scale fee with our intern and fellow clinicians. Please inquire to ask on these rates.

Payments

All payments (copays, deductibles, out of pocket fees, and sliding scale fees) are due at time of service.

Cancellation Policy

Accepted Answer

All appointments that require to be cancelled or rescheduled must be done 24 hours prior (for 3pm and earlier appointments) or 48 hours prior (for 4pm and later appointments). Failure to do so will result in a late cancellation fee.
